For the comment about the doctor and river being a couple, they are not. In the episode near the beginning of the series, when the doctor drops river back off at the prison, she kisses him, and asks him if they had ever done that before, he said no, she said that it wouldn't happen again, as if they only kiss the once and that was it. It implied that they weren't a couple.
I had a feeling that she was who she is at the beginning of the episode, and I may be in the minority here but I liked the idea. The fandom has had so many ideas on who she could be since we first met her, and nobody came up with this one.
