Having Romana back would be great although I'm not entirely happy with Alex Kingston. Maybe it's because how her character developed in ER and I'm just prejudiced but I can't stand her...
As far as I remember it was never stated on-screen that Romana actually left E-space (it seemed pretty much impossible when she decided to stay that she would ever be able to come back). That would explain how she survived the Time War. And if she only came back after the walls between the universes started to break down during series four that would explain as well why Nine didn't sense her. As for Ten, maybe his experiences on the planet of the Ood had something to do with the fact that he didn't recognize her? (If you scratch the last one and a half sentence I've had this theory for about half a year now - to explain how Romana showed up in Pete's World in my current WIP).
At least the Doctor and Romana eventually marrying wouldn't be totally unlikely. The chemistry was there with Four and Romana II as well. But the Rani? Ugh - I really don't want to think about that...
